Mike Vernon putting his head back on.
Reminds me of Terminator 3. Arnold's T-800 re-attaching its' own head after being decapitated during a fight with the rival T-X.

Another card from the 1995-96 UD set...
Looks like Mike Richter's mask is rolling around behind the net.
Although it's a new card to my collection, the photo seemed familiar... His Collector's Choice card from the same year also features him without a mask...

With the same ad on the opposing team's arena boards, the photos are likely from the same sequence.
In today's game, I'm pretty sure the ref blows the whistle to stop the play if the goaltender loses his mask.
